            John Arsenault spent his last night at First Congregational =
Church's cold-weather shelter. On the evening of Friday, March 3, he =
came by taxi to the shelter from Concord Hospital. Arsenault, 67, could =
barely walk. He needed help signing his name. Shelter guests and =
volunteers sat with him, bringing him water, making sure he was =
comfortable.

            Six hours later, Arsenault collapsed. He died at Concord =
Hospital shortly after.=20

            Arsenault's death - which the Medical Examiner's Office =
ruled "natural" - left more questions than answers. His family and =
friends still wonder about the circumstances of Arsenault's discharge =
from Concord Hospital, which only handles medically acute cases.=20

            But amid the questions, a portrait of his life, of his =
interests and quirks and loved ones, has taken shape. And from =
Arsenault's final weeks of life, another, larger tale has emerged: of a =
health care system that struggles to accommodate patients who aren't =
medically acute - but whose behavioral issues prove too burdensome for =
caretakers to handle alone.=20

            "I didn't know if something medically was wrong. I just knew =
the behavior part of it wasn't right," said Diane Briggs, who helped =
care for Arsenault. But "he didn't fit a criteria for being admitted," =
she said. "I'm not blaming anybody for his death. . . . The people I =
dealt with were caring and decent."=20

            "But there needs to be something set in place for people who =
aren't acute," she said.=20

            Briggs and June Williams, Arsenault's niece, didn't know if =
Arsenault ever received a behavioral diagnosis. But "he has always just =
been a little bit slow. His mentality was a little bit young. I just =
considered John to be a little quirky, a little different," Briggs said. =
Arsenault lived on a Loudon farm with his cousin, James Brown, and =
Brown's family. Briggs is engaged to Brown's son and has lived on the =
farm for the past six years.=20

            Two weeks before his death, Arsenault's behavioral issues =
multiplied. He was, Briggs said, "even more quirky than usual." She took =
him to the hospital. She called dozens of organizations and health care =
providers. But Briggs couldn't find Arsenault the care she felt he =
needed. Someone suggested Briggs put Arsenault in a shelter; she =
refused.=20

            On March 10, she took him to Capital Region Family Health =
Center, a department of Concord Hospital. She was frustrated, worn out =
from caring for Arsenault, her five children and two grandparents. =
Arsenault, she felt, needed a thorough evaluation. Someone from the =
center told Briggs she could leave Arsenault in their care. Briggs left =
her phone number. "I was promised that he'd be taken care of, and he'd =
be in a safe place," she said.=20

            Briggs is piecing together what happened next. She knows =
Arsenault went to the hospital emergency room; a friend saw him there. =
But that evening, Arsenault turned up at First Congregational. Shelter =
workers had been informed that he was cleared to leave the hospital. =
When his legs gave out around midnight, volunteers summoned an =
ambulance.=20

            Briggs said she didn't learn Arsenault had been discharged =
until after his death. "If they had called me and said, 'We just can't =
admit him,' I would have gone and got him," she said. "I think they =
thought I was so exhausted. . . . I think there was so much =
miscommunication from beginning to end."=20

            Hospital staff wouldn't comment on the circumstances of =
Arsenault's arrival at the center or about his discharge from the =
hospital because of patient confidentiality agreements. But "Arsenault =
received excellent clinical care and compassionate support at Concord =
Hospital, not only at the time of his death but on many occasions before =
the last days of his life," Michael Green, the president and CEO of the =
hospital wrote to the Monitor.=20

            "The doctors at Concord Hospital, no matter how good they =
are, are not clairvoyant. They cannot always predict when, how fast or =
how far that condition will deteriorate," Green wrote.=20

            Briggs was told Arsenault's death was heart-related. "They =
said it had been there for years." She doesn't know if Arsenault's heart =
condition caused his recent behavior.=20

            'He was childlike himself'=20

            Though Arsenault spent his last night at a shelter, he had a =
home. "I want people to know that Jonathan was very loved," Briggs said. =
"He had a very complete life."=20

            He loved taking photographs, playing board games and =
cooking, especially chicken liver. And "he loved animals," Briggs said. =
"If he could take in every stray in the world, he would." Briggs is =
still feeding the stray cat Arsenault cared for.=20

            Arsenault didn't start out in New Hampshire. He grew up in =
Massachusetts with his brother and sister. He worked in Boston for =
decades: at a book publishing company, a local restaurant and his aunt =
Laura Brown's ice cream shop, said Williams, whose mother, Sarah =
Mallett, is Arsenault's sister. For a time, he lived on his own, "I =
think pretty successfully," Williams said.=20

            But he was very close to Laura Brown. When she closed her =
ice cream shop and retired to Loudon, Arsenault went along. For a time, =
he worked as a janitor. He never drove; he rode his bike everywhere. =
When Brown died, Arsenault moved to the farm.=20

            He took to rural life: helping Briggs ready the greenhouse =
for planting, working for hours in the fields. Sometimes he and Briggs =
had tomato fights. He loved Briggs's kids; he cheered them on at sports =
games, played board games with the youngest, let Briggs's daughter give =
him dance lessons. "He was childlike himself," Briggs said. "I think it =
kept him young."=20

            Briggs misses the little things, even the ones that =
frustrated her at the time. Like how Arsenault wore work boots to go =
jogging, and hid tools and groceries. Or the special, wide smile he'd =
make by scrunching his lips. Or watching American Idol together. "His =
