CMS Issues New Real Choice Systems Change Grants

I.  The FY 2004 Real Choice Systems Change Grants Notice of Funding
Availability is available (CMS-2189-N) on the GPO web site. There are
nine grant opportunities available under this notice.  They are:

A. Quality Assurance & Quality Improvement in Home and Community Based
Services (HCBS)
B. Integrating Long Term Supports with Affordable Housing
C. Portals from Early Periodic Screening, Diagnosis, and Treatment
(EPSDT) to Adult Supports
D. Comprehensive Systems Reform Effort
E.  Mental Health: Systems Transformation
F.  Rebalancing Initiative
G.  Living with Independence, Freedom, and Equality (LIFE) Account
Feasibility and Demonstration
H. Family-to-Family Health Care Information and Education Centers
I.  National State-to-State Technical Assistance Program for Community
Living

II. The  FY 2004 Real Choice Systems Change Grants solicitation package
is available on the CMS web site.  Go to
http://www.cms.hhs.gov/newfreedom/ and click on the link that says
"SOLICITATION: 2004 Real Choice Systems Change Grants"  (At the moment
it is under the broad heading of "Real Choice Systems Change Grants in
2003" but that will be fixed soon.)

III.  An applicants teleconference will take place Monday, June 7, 2004
from 3-5 pm Eastern.  In order to connect and register with the operator
for the call, begin calling at 2:45 p.m.  The dial in number is

Rama.
